It takes a village to keep patients out of the hospital and our system is built to be the single source of truth for all partners to view the most recent medications, allergies, labs, home monitoring data and any relevant care notes about patients. For patients that we care for, we are able to easily gather needed information for our partners to keep patients out of the hospital. Our system is HIPAA compliant and hosted on Amazon web services. Partners must sign a BAA that have access to patient level information prior to joining our platform.
